Qualifications
A.D.N degree from an accredited school of nursing is required, with a preference for B.S.N. Alternatively, a three-year diploma from an accredited program can be substituted for a nursing degree.
Current Registered Nurse (RN) license in the state of practice is required.
You must maintain continuing education credits as required by the state of practice.
We value a minimum of five (5) years of experience in clinical nursing, as it is essential for this role.
Preferably, you possess a minimum of three (3) years of experience in renal nursing, which would be highly advantageous.
Previous Case Management experience is required, as it contributes to success in this position.
You have demonstrated experience and effectiveness in a change agent role, showing your ability to adapt and drive positive transformations.
Knowledge and understanding of CQI (Continuous Quality Improvement) techniques is important for this role.
Healthcare performance coaching experience is necessary, enabling you to support and guide our team effectively.
Current CPR certification is mandatory to ensure the safety and well-being of our patients and colleagues.
Possession of a valid driver's license with a positive driving record is required, as well as meeting the necessary insurance coverage requirements.
Basic computer skills, including proficiency in MS Word and Outlook, are essential for effective communication and documentation.
Within 90 days of employment, you will need to demonstrate functional proficiency with DaVita specific clinical software programs, including Capella.
To work seamlessly with our team, a home office with internet connectivity at a minimum of 1MB upload and 1MB download speed is necessary.
