Responsibilities
Provide personalized care management services to beneficiaries during planned admissions to acute care hospitals or during transitions from case management or disease management programs.
Conduct comprehensive assessments to identify the specific needs and requirements of patients with acute, real-time care needs.
Develop and implement individualized care plans, ensuring that all necessary interventions and services are in place to bridge care gaps effectively.
Collaborate with healthcare professionals, including physicians, nurses, and social workers, to coordinate and streamline patient care.
Monitor and evaluate patients' progress, making necessary adjustments to care plans as required to achieve optimal clinical outcomes.
Advocate for patients, ensuring their needs are met and their rights are respected throughout the care process.
Educate patients and their families about available resources, treatment options, and self-care techniques to promote long-term well-being.
Stay updated on the latest advancements in healthcare practices and policies, integrating them into care management strategies.
Qualifications
Education/Experience: Graduation from a Nursing program is required, with a preference for candidates holding a BSN degree. Alternatively, graduation in Clinical Psychology or Clinical Social Work will also be considered.
Experience: A minimum of 2 years of relevant experience is required, demonstrating proficiency in care management and patient advocacy.
License/Certification: Must possess and maintain a current, valid, and unrestricted Registered Nurse license.
