Role Overview
As a Chronic Care Manager (Registered Nurse), you will play a crucial role in our Care Team, working alongside Caregivers and Care Coordinators to ensure our clients receive the highest level of care and support. You will have the opportunity to make a real difference in the lives of those we serve by providing comprehensive nursing services and promoting a holistic approach to health and well-being.
Responsibilities
Collaborate with other members of the Care Team, performing supervisory visits as required, and monitor clients' conditions with the support of the Care Coordinator and Caregiver Team.
Conduct staff in-service training and deliver education specific to client care, fostering a culture of continuous learning and professional development.
Conduct thorough assessments prior to client admission to ensure their needs will be met with the utmost attention and care.
Craft and document comprehensive care plans, reviewing and adapting them as needed to ensure personalized and effective care.
Maintain accurate and up-to-date client charts and nursing notes, ensuring meticulous record-keeping to provide seamless and high-quality care.
Manage medication orders issued by licensed prescribing practitioners, maintaining precise records and adhering to company protocols.
Educate the Care Team, clients, and their families about potential medication side effects and adverse reactions, empowering them with knowledge and understanding.
Foster effective communication within the Inter-Disciplinary Clinical Team, proactively addressing changes in clients' conditions and making referrals when necessary.

Desired Skills And Experience
You are a registered and licensed RN in the State of Illinois and maintain current and unrestricted licensure.
You possess a minimum of two years of experience as a Nurse in either a Home Care or Hospital setting, bringing valuable expertise to our team.
Your knowledge of quality programs in healthcare settings reflects your commitment to excellence in care delivery.
With your natural leadership and supervisory skills, you inspire and guide others to deliver outstanding care.
Your experience in maintaining clinical records in compliance with regulatory requirements ensures our clients' care is well-documented and meets all standards.
Your excellent presentation and communication skills, both verbal and written, enable you to connect effectively with our team and the clients we serve.
