In this role, you will play a crucial part in providing exceptional chronic care services to our valued Primary Care patients, focusing on wellness and disease prevention.
Responsibilities
As a Chronic Care Management Nurse, you'll be the linchpin in our mission to enhance patient satisfaction, elevate the quality of patient outcomes, ensure safety, and reduce adverse events, hospitalizations, and emergency room visits. Your responsibilities will include:
Collaborating seamlessly with healthcare providers and patients who are managing chronic conditions such as diabetes, hypertension, asthma, obesity, cardiac risk reduction, and stress reduction.
Championing positive behavioral changes that support wellness and medication adherence.
Empowering patients to self-manage their chronic conditions by setting goals, utilizing effective teaching methods, and fostering patient advocacy within the broader community.
Fulfilling Primary Care nurse duties, encompassing essential tasks like assisting patients with prescription needs and refills, conducting triage including telephone encounters, and coordinating services involving families and external organizations.
Advocating for our patients by facilitating access to financial assistance and initiating referrals to community-based organizations such as Home Care, Healthy Start, Dental, and Behavioral Health.
Continuously evaluating patient care guidelines, identifying opportunities for improvement, and providing education where needed.
