Registered Nurse (Rn) - Remote | Wfh

Responsibilities
Patient Assessment and Care Planning
Conduct comprehensive initial assessments of patients and their families to identify their home care needs.
Perform thorough physical assessments and gather detailed medical histories.
Utilize health assessment data to determine nursing diagnoses.
Develop personalized care plans that establish goals based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative nursing actions.
Collaborate with patients and their families, involving them in the care planning process.
Nursing Procedures and Medication Administration
Implement appropriate preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed by physicians.
Provide clear and concise health care instructions to patients, ensuring their understanding and compliance.
Case Management and Communication
Act as a dedicated Case Manager, coordinating patient care for assigned caseload when assigned by the Clinical Supervisor/Nursing Supervisor.
Identify discharge planning needs and implement strategies accordingly.
Prepare clinical notes and update primary physicians regularly, ensuring smooth communication and continuity of care.
Communicate patient needs and report any changes in their condition to physicians promptly.
Obtain/receive physician's orders as required to ensure optimal patient care.
Minimum Qualifications
Active Registered Nurse License in the State of California with at least 1 year of professional RN experience.
Previous experience in Home Health (RN) is a plus.
Strong communication skills, with the ability to effectively interact with patients and their families.
Bilingual communication skills are a plus.
Reliable transportation, current driver's license, and auto insurance.
Current CPR certification.
Ability to perform the physical aspects of the position.
Completion of TB test and physical examination.
