Job Description
Essential Functions
As a Care Coordinator, you will play a vital role in providing exceptional care to our clients. Your responsibilities will include:
Client Matching: Actively participate in screening and matching Home Care Assistants (HCA's) with our valued clients.
Scheduling: Skillfully schedule matched HCA's and clients while efficiently responding to changes in schedules to ensure seamless coverage for all clients. Maintain the ongoing schedule with care and precision.
Training Support: Collaborate with our Human Resources department by assisting in the initial orientation and training of new HCA's, as well as aiding in their continuous development and training.
Assessment and Planning: Conduct comprehensive client needs assessments and formulate customized plans of care to ensure our clients receive the highest quality of support.
Home Visits: Conduct regular home visits to our clients to uphold the standard of care and adherence to their individual plans.
Communication: Maintain effective and timely communication with HCA's regarding client care issues. Offer guidance, coaching, and support to HCA's as required, fostering a collaborative care environment.
Transportation Coordination: Coordinate suitable transportation for clients and HCA's to appointments, outings, and errands, ensuring their safety and convenience.
Additional Support: Provide additional support to both clients and HCA's as needed to enhance their well-being and overall experience.
Preferred Qualifications
To excel in this role, the following qualifications are preferred:
A college degree in a healthcare-related field such as health care, social work, gerontology, or a similar discipline.
Prior experience in healthcare scheduling, demonstrating your ability to efficiently manage schedules.
Required Qualifications
To be considered for this position, you must meet the following requirements:
Access to reliable transportation and a clean driving record, as transportation is a crucial aspect of this role.
A minimum of two years of experience working with the geriatric population or in a healthcare setting, showcasing your dedication to senior care.
Submission of at least two professional references along with your application.
Successful completion of a background check.
Proof of a negative TB PPD test or chest x-ray to ensure the safety of our clients and team members.
