Nurse Case Manager - Remote | Wfh

We are seeking a caring and compassionate Care Manager to join our team! As a Care Manager, you will work closely with our members and their families/caregivers to develop and implement individualized care plans that promote their health and wellbeing. Your role will be critical in ensuring that our members receive the highest quality of care, and that they feel supported and cared for throughout their healthcare journey.
Responsibilities
Work with our members and their families/caregivers to develop and update individualized care plans that are tailored to their specific needs and goals
Implement care plans and coordinate the delivery of covered services to ensure that our members receive the best possible care
Maintain ongoing communication and collaboration with our members, their families/caregivers, and their healthcare providers to ensure that our members' needs are being met and their care is being properly managed
Monitor the provision of services by our network providers to ensure that they are appropriate and aligned with our members' care plans
Conduct ongoing monitoring of our members' health, safety, and functional status, and track their progress towards their established goals
Facilitate care transitions, including discharge and transition planning from hospitals or nursing homes, to ensure that our members receive the best possible care throughout their healthcare journey
Determine member eligibility for our program and quality initiatives, and ensure that our members receive appropriate Advance Care Planning and Health Care Proxy administration
Review members' medical status and issues, and identify follow-up issues that may require attention
Serve as the primary point of contact with our members' healthcare providers, including PCPs and specialist physicians, as well as their long-term care service providers
Monitor members' medication adherence in conjunction with the RN and Care Management Supervisor, and provide education/coaching in self-management of disease processes
Conduct monthly outreach to our members to check on their status, and update their case records accordingly
Participate in Quality Assurance and Improvement activities as directed by the Director of Care Management
Perform any other duties as assigned by the Care Management Supervisor and/or the Director of Care Management
