Hiv Prevention Health Educator - Remote | Wfh

We are currently seeking a passionate and dedicated individual for a significant role in the field of Primary Care HIV Prevention (PCHP). This program, generously funded by a reputable health organization, is dedicated to reducing the risk of HIV transmission, with a special focus on facilitating access to and utilization of pre-exposure prophylaxis (PrEP). The heart of the PCHP initiative is to enhance the diagnosis, treatment, and prevention of HIV within our community.
As a PCHP Health Educator, you will have the opportunity to work closely with the communities we serve. Your role will involve providing vital HIV education and health promotion services, conducting community outreach, administering both on-site and off-site HIV testing, and ensuring smooth linkages to care. Your contribution will be pivotal in counseling, testing, and directing at-risk individuals toward essential HIV, sexual health, and primary care services. Additionally, you will be instrumental in building and nurturing relationships with community partners.
Essential Duties And Responsibilities
Develop and implement targeted outreach strategies, incorporating both traditional methods and online platforms, such as educational seminars, testing events, and informative tabling sessions within the program's service area.
Engage both patients and non-patients through informative health education workshops, HIV screenings, empathetic counseling, and promotions related to sexual health.
Conduct HIV rapid testing, ensuring compassionate delivery of results and swift connections to necessary supportive services for clients.
Foster the adoption of harm reduction strategies, including advocating for PrEP/PEP, distributing condoms, and delivering patient education.
Establish and nurture partnerships with local schools, places of worship, community housing organizations, and various community entities.
Collaborate in the creation of educational materials for patients and staff, spanning social media posts, newsletters, and patient-centric resources.
Effectively deliver educational content, utilizing harm reduction techniques and evidence-based information, both internally and at public engagements.
Work closely with Care Teams to ensure comprehensive, high-quality care is extended to patients.
Assist the Program Manager in identifying and addressing gaps in HIV testing, linkage, and retention services.
Actively contribute to dismantling barriers to clinical and care services by advocating for HIV prevention as an integral part of primary care.
Maintain accurate and up-to-date patient records, meticulously tracking contacts, counseling sessions, testing outcomes, referrals, and outreach achievements.
Participate in constructive staff meetings and engage in continuous professional development training.
Undertake special projects and fulfill other job-related responsibilities as assigned.
Minimum Qualifications
Possession of an Associate's/Bachelor's Degree in Health Education, Public Health, Social Work, or a related field is preferred. Alternatively, a minimum of two years of work or volunteer experience in a patient-centered field is acceptable in lieu of a degree.

A minimum of 2 years of experience in HIV/AIDS prevention, outreach, and health promotion is preferred.
Experience working with People Living with HIV/AIDS (PLWHA), LGBTQIA+ individuals, and diverse communities is highly valued.
Proficiency in MS Word, Excel, and PowerPoint, as well as familiarity with electronic medical records, is advantageous.
Demonstrated proficiency in organizational skills, interpersonal interactions, verbal and written communication, and the ability to manage multiple tasks concurrently.
Comfort and capability in both independent and collaborative work settings, including remote environments.
Cultural competency and ease in engaging with LGBTQ populations and communities of color.
Bilingual fluency in English and Spanish is a plus.
