Job Description
We are seeking a dedicated and compassionate individual to join our team as a Client Services Coordinator. As a Client Services Coordinator, you will play a vital role in providing essential support to our clients and their caregivers. Your responsibilities will involve maintaining a caseload of ongoing and short-term clients, conducting assessments, and creating personalized care plans. You will also have the opportunity to offer guidance, coordinate with caregivers, and facilitate support groups to enhance their well-being.
Responsibilities
Maintain a caseload of ongoing and short-term clients, ensuring their needs are met effectively.
Conduct thorough assessments and develop personalized care plans for each client, fostering their holistic well-being.
Engage in timely follow-ups with both clients and providers, ensuring seamless communication and support.
Conduct home visits as necessary, enhancing our clients' comfort and convenience.
Collaborate in program development, contributing your valuable insights to enhance our services.
Provide compassionate and personalized case coordination and counseling to caregivers.
Offer direction and assistance on various caregiving matters, such as coping strategies, long-term care planning, advocacy, and referrals.
Lead support groups and educational training sessions, creating a supportive community for caregivers.
Maintain accurate case records in our database, documenting all caregiver services provided.
Identify and curate up-to-date resources for clients, ensuring they have access to the best support available.
Collaborate with community partners to expand our network of support.
Promote our agency and programs by actively participating in outreach efforts, community events, and public representation.
Develop and nurture relationships with external organizations to foster collaborative outreach and partnership opportunities.
Utilize social media and other digital tools effectively to reach a diverse audience and raise awareness.
Qualifications And Experience

Bachelor’s degree in a relevant field such as Social Work, Counseling, or Psychology.
Exceptional interpersonal and organizational skills that drive effective client engagement.
Proficiency in Microsoft Office and excellent office management and computer skills.
Strong written and oral communication skills, allowing you to connect with clients and colleagues effectively.
