Job Description
We are seeking a dedicated Healthcare Care Coordinator to join our dynamic team, working in a compassionate and patient-centered environment. In this role, you will play a crucial part in ensuring the well-being of our members at designated HIV primary care facilities. Your responsibilities will include coordinating care, providing health education, and supporting members in reaching their health goals. As a Healthcare Care Coordinator, you will make a meaningful impact on the lives of our HIV-positive, transgender, and homeless members, fostering a healthier community for all.
Responsibilities
Provide telephonic care management services to members, offering care coordination, transition of care, out-patient follow-up, and review of ancillary services to enhance health outcomes.
Develop and implement personalized member Plans of Care, collaborating with members, family caregivers, primary care providers, and care managers to align health coaching and care coordination activities with member needs and preferences.
Promote accessibility to comprehensive, high-quality healthcare services for HIV-positive, transgender, and homeless members.
Deliver health education and coaching on various health topics, including HIV disease, risk reduction, and chronic illnesses like hepatitis C, diabetes, and hypertension.
Collaborate with primary care providers, community care managers, and other relevant stakeholders to ensure coordinated and comprehensive member care.
Analyze utilization trends and potential member needs using encounter data, pharmacy records, and health assessment tools.
Document member goals, interventions, and care coordination activities in compliance with professional standards and regulatory guidelines.
Support and mentor Health & Wellness Advisors, contributing to their growth and competence.
Participate in departmental case conferences, reviews of Plans of Care, and recommend improvements to enhance member well-being.
Assist other departments in resolving member concerns related to retention, utilization management, quality, customer service, and provider relations.
Attend relevant in-service training and external education sessions as directed by the department.
Conduct clinically focused member assessments, health coaching, and medication reconciliation to improve quality measures and clinical outcomes.
Engage in special projects such as audit reviews, quality improvement initiatives, community health education, and promoting our health plan.
Minimum Qualifications
Bachelor’s degree in a healthcare-related field from an accredited college or university; Master’s degree in Nursing, Social Work, or a related discipline preferred.
At least two years of clinical experience in HIV care or support systems, including care coordination, health education, and care management. Managed care experience is a plus.
Proficiency in Microsoft Office products and computer literacy.
Bilingual skills (English/Spanish preferred); fluency in other languages welcomed.
Valid New York State license as a registered professional nurse.
Proof of COVID Vaccination.

Competitive salary range of $96,000 to $96,500, plus excellent UNION Benefits and PENSION Plan.
