Responsibilities
Engage in both one-on-one and group outreach presentations to recipients and community members, fostering a sense of inclusivity and understanding.
Articulate program education and enrollment choices to recipients, ensuring they have the information they need to make informed decisions.
Guide recipients through the realm of managed care options, providing assistance in utilizing healthcare and dental plans effectively.
Cultivate and maintain positive relationships with county staff, state agencies, community groups, and recipients, fostering a collaborative and supportive environment.
Execute accurate data entry into designated internal automated systems, adhering to project policies and procedures.
Perform outreach activities such as phone calls, home visits, and community presentations, following project guidelines and protocols (constituting 75% of your role).
Develop an in-depth understanding of the community landscape and actively participate in community events, health fairs, and related activities as necessary.
Collaborate with fellow Outreach Counselors and team members, offering assistance and support to accomplish daily responsibilities effectively.
Requirements
Possess a High School diploma / GED, demonstrating your commitment to education and personal growth.
Equipped with a Windows 10 Laptop featuring video capabilities and a USB headset; essential if the position is temporarily remote / hybrid / remote.
Provide 2 professional references, preferably from supervisors or managers, including their active contact information, showcasing your professional relationships and reputation.
Flexibility to undertake travel within the county, further enhancing your engagement with the community.
