As an Outreach Counselor, you'll play a crucial role in presenting program education, managing care options, and fostering positive relationships with stakeholders. Responsibilities
Engage in both one-on-one and group outreach presentations to recipients and community members, ensuring clear communication and understanding.
Present a range of program education and enrollment choices to recipients, empowering them with the knowledge they need to make informed decisions.
Introduce managed care options to recipients, offering guidance on navigating healthcare and dental plans effectively.
Cultivate and maintain positive relationships with county staff, state agencies, community groups, and recipients, fostering a harmonious working environment.
Accurately enter data into designated internal automated systems following project policies and procedures.
Conduct outreach activities, including phone calls, home visits, and community presentations, adhering to project policies and procedures (constituting approximately 75% of work time).
Establish and enhance your understanding of the local community, actively participating in community events, health fairs, and other relevant activities.
Collaborate with fellow Outreach Counselors and team members to manage daily tasks efficiently.
Minimum Requirements
Possess a High School diploma or GED, demonstrating a solid educational foundation.
Have access to a Windows 10 Laptop with video capability and a USB headset (essential for remote, hybrid, or temporarily remote positions).
Provide two professional references, preferably from a supervisor or manager, including their active contact information.
