Out Of Network Case Manager, Rn - Remote | Wfh

We are seeking a skilled and experienced Case Manager with a passion for helping others to join our team. As a Case Manager, you will play a critical role in coordinating and managing patient care, ensuring that they receive the best possible treatment and support.
Responsibilities
Conduct assessments of patient needs, develop and implement care plans, and monitor patient progress.
Maintain accurate documentation and comply with legal guidelines and documentation standards.
Collaborate with interdisciplinary teams to achieve goals and objectives.
Work independently and utilize team concepts to accomplish goals.
Requirements
Possess excellent communication and assessment skills.
Type at least 40 words per minute and be proficient in Microsoft Excel and Word applications.
Knowledge of Microsoft Access is desirable.
Formal training indicated by a high school diploma or equivalent.
Must have current California State Registered Nurse licensure and a minimum of 5 years clinical experience.
Proficiency in preparing complex reports and analysis.
Must possess a valid, unrestricted California Driver's License and auto insurance.
Preferred
Experience in case management.
Managed care experience is helpful.
