MyCare Medical is seeking a highly experienced and dynamic individual to join our organization as a Senior Director of Care Management.
As the Senior Director, you will play a crucial role in leading and overseeing the care management operations and strategies to ensure the delivery of high-quality, patient-centered care. You will collaborate with cross-functional teams, including healthcare providers, administrators, and other key stakeholders, to optimize care coordination and improve patient outcomes.
In this role, you would be responsible for (not limited to)
Strategic Leadership
- Provide visionary leadership and guidance to the care management team, ensuring the delivery of exceptional care services.
- Identify areas for improvement and implement innovative care management initiatives to enhance patient care and outcomes.
- Develop and execute the overall care management strategy aligned with organizational goals and objectives.
Care Management Operations
- Drive continuous improvement initiatives to enhance care coordination, utilization management, and discharge planning processes.
- Establish and monitor key performance indicators (KPIs) to measure the effectiveness and efficiency of care management processes.
- Oversee the day-to-day operations of the care management department, ensuring efficient workflows and adherence to regulatory requirements.
Staff Management and Development
- Recruit, train, and mentor a high-performing team of care management professionals.
- Conduct regular performance evaluations and implement performance improvement plans as needed.
- Provide ongoing guidance and support to team members, promoting their professional growth and development.
Collaboration and Stakeholder Engagement
- Foster strong relationships with key stakeholders and participate in relevant committees, task forces, and quality improvement initiatives.
- Collaborate with healthcare providers, clinicians, and administrators to ensure seamless care transitions and effective communication across the continuum of care.
- Engage with external partners, such as insurance companies and community organizations, to optimize care coordination and resource utilization.
Quality Assurance and Compliance
- Develop and maintain policies and procedures to support consistent and evidence-based care management practices.
- Ensure compliance with regulatory standards and guidelines related to care management, privacy, and patient rights.
- Monitor and analyze care management outcomes, identify trends, and implement interventions to improve quality and efficiency.
We are seeking someone who possess the following qualifications to be considered for the Senior Director of Care Management opportunity
- Strong understanding of healthcare regulations, compliance, and accreditation standards.
- Extensive experience in care management, case management, or related roles, with progressive leadership responsibilities.
- Bachelor's degree in nursing, healthcare administration, or a related field (Master's degree highly preferred).
- Proficient in data analysis, performance metrics, and quality improvement methodologies.
- Demonstrated ability to develop and implement strategic initiatives and drive organizational change.
- Excellent leadership, communication, and interpersonal skills.
- In-depth knowledge of care coordination principles, utilization management, discharge planning, and population health management.
- Possession of relevant certifications (e.g., Certified Case Manager, Certified Care Manager) is advantageous.
