We are seeking a passionate and dedicated Health Educator to join our dynamic team at a leading healthcare organization. As a part of the Primary Care HIV Prevention (PCHP) program, you will play a vital role in reducing the risk of HIV transmission and improving access to essential care. Your impact will be felt within the vibrant communities we serve, helping to educate, promote health, and create lasting connections.
Responsibilities
Empower communities with targeted outreach, including engaging educational seminars, testing events, and community outreach initiatives.
Conduct health education workshops, HIV screenings, counseling sessions, and sexual health promotions to provide comprehensive support to our patients and the community.
Offer HIV rapid testing with empathy, ensuring prompt and compassionate connection to crucial supportive services.
Advocate harm reduction strategies such as PrEP/PEP, condom distribution, and patient education to foster a healthier community.
Establish and nurture partnerships with local schools, places of worship, community organizations, and more, expanding the reach of our mission.
Collaborate with Care Teams across our network to deliver exceptional, holistic care to patients throughout our clinics.
Work alongside the Program Manager to identify and address service gaps related to HIV testing, linkage, and retention.
Break down barriers to clinical and care services, ensuring HIV prevention is seamlessly integrated into primary care.
Create compelling educational materials, utilizing evidence-based information and harm reduction approaches, to educate both internal staff and the public.
Keep accurate and up-to-date patient records, tracking contacts, counseling, testing, referrals, and outreach performance.
Participate in enriching staff meetings and development trainings to continuously enhance your skills and knowledge.
Embark on special projects and other duties that align with our mission and community needs.
Qualifications
Associate’s/Bachelor’s Degree in Health Education, Public Health, Social Work, or a related field preferred. Alternatively, a minimum of two years of relevant work or volunteer experience with patient contact.
