We are seeking a Navigator/Pediatric Care Coordinator who will be responsible for connecting children, youth, and families in schools to integrated health care services. As a Navigator, you will guide clients through eligibility, enrollment, and program specifications for various health plans to ensure they receive the care they need. You will follow the Navigation Workflow to assess clients and provide clinical interventions and treatments. In addition, you will help address barriers to services, connect clients to other internal and external social services resources, and advocate for their basic needs. Your role will also include clinical support duties such as recording patient information and assisting physicians during examinations, as well as outreach and education activities such as presenting at community events and preparing and disseminating educational material.
Responsibilities
Navigation:
Stay up-to-date with eligibility, enrollment, and program specifications for various health plans to help clients select plans.
Register, collect and input client data into a clinical database, and develop monthly and weekly reports.
Participate in care coordination and/or case management to help address barriers to services.
Provide and track follow-up on health and social service encounters with individuals, families, and/or community groups.
Connect individuals to other internal and external social services resources and advocate for clients’ basic needs.
Inform and engage staff and peers about community assets and community challenges.
Collect and manage daily cash intake and generate daily cash reports.
Ensure the protection and security of personally identifiable confidential information professionally and responsibly.
Participate in program quality control meetings with the school setting staff, stakeholders, and the PVHC team.
Manage patient assistance program, prior authorization, and verification of insurance benefits.
Clinical Support Duties
Record patient height, weight, blood pressure, pulse, and temperature.
Assist physician while the patient is being examined.
Interpret simple laboratory results.
Sterilize instruments and clean equipment.
Give instructions to the patient on COVID, flu, strep UA collection procedures as indicated by a physician.
Print referrals to other providers, ancillary services, organizations, or hospitals as indicated.
Ensure that exam rooms are clean and stocked and maintain daily inventory and orders supplies.
Assess patients for appropriate sample according to procedure.
Collect samples in appropriate containers after positive patient identification.
Properly label specimen(s) and complete patient according to established criteria to ensure a positive patient outcome.
Verify the identity of samples with orders and process laboratory specimen(s) as indicated.
Record and scan lab results into EMR.
Route specimens to the appropriate test site.
Respond courteously and efficiently to all scheduled patients in the EMR appointment system as indicated and processes patients.
Verify current patient information in the EMR system and enter pertinent patient information into the system.
Upon request, prepare a daily schedule of patient appointments and notify Provider of changes.
Upon request, coordinate information pertaining to prescriptions, refills, lab reports, etc. with physicians and patients.
Call prescriptions to the pharmacy for the provider.
Scan all mandatory forms into EMR.
Address bump list to keep patient scheduling current.
Outreach & Education
Identify and engage individual community members, families, groups, and/or partner agencies in school settings for PVHC programs.
Participate and present at community events such as health fairs, local agencies, coalitions, and partner agencies.
Prepare and disseminate educational, promotional, or other informational material to support program goals.
Educate individuals and school community stakeholders about program structure.
Requirements
Certification: Community Health Worker/Promotor (a) State of Texas, college credits, or equivalent experience.
Certification: Certified Application Counselor for the Health Insurance Marketplace program.
Previous work experience in Open Enrollment.
Bilingual 100% (English/Spanish a must).
