We are seeking a dedicated and empathetic individual to join our team as a Field-Based Mental Health/Care Manager. As a valued team member, you'll be responsible for providing intensive and structured services to our clients while coordinating with our talented treatment team.
Your Day-to-day Will Involve
Offering compassionate field-based mental health and case management services to adults in need.
Collaborating with our dynamic treatment team to implement customized treatment plans.
Providing safe transportation for clients to appointments and multiple sites.
Participating in team conferences to ensure our clients receive integrated and comprehensive care.
Maintaining meticulous billing records and ensuring timely submissions.
Keeping client files and documentation up-to-date following the guidelines set by the Los Angeles County Department of Mental Health.
Responding to on-call crises during scheduled rotations, making a significant difference in people's lives.
To Succeed In This Role, You'll Need
Legal authorization to work in the United States.
Your reliable transportation, a valid California Driver License, and proof of automobile insurance coverage.
Excellent writing and oral communication skills, complemented by strong interpersonal abilities.
Outstanding time management skills and an exceptional attention to detail.
Full COVID-19 vaccination with eligible booster(s).
Bilingual capability in Spanish and API Languages would be a plus, but it's not required.
Ideally, at least one year of direct service experience in a hospital or mental health setting with an interdisciplinary team approach.
