Job Overview
As a Chronic Care Navigator, you will play a crucial role in implementing, managing, and coordinating our Chronic Care Management Program activities. Your primary focus will be on directly engaging, managing, and triaging complex clinical needs to ensure our patients receive the best care possible.
Responsibilities
Develop a deep understanding of chronic pain and family medical care requirements for optimized, coordinated population health.
Proactively manage and support patients via text using our innovative Phamily platform.
Conduct proactive patient management through personalized phone calls.
Expertly triage and assess patient health status to provide timely interventions.
Facilitate seamless coordination of care between patients and healthcare providers.
Conduct active reviews of patient charts to assess health needs and identify care gaps.
Prioritize delivering a positive patient experience through courteous and empathetic digital interactions.
Accurately and expeditiously route patient escalations to ensure timely resolution.
Make referrals to appropriate staff when necessary to enhance patient care.
Requirements
Reside in Alabama (formally).
Hold certification as a Registered Medical Assistant (RMA), Certified Medical Assistant (CMA), or Certified Clinical Medical Assistant (CCMA).
Ideal Candidates Will Have
Certification as a Medical Assistant.
A High School diploma.
2 years of Internal Medical experience (a plus).
