At The Door, youth can access health care and education, mental health counseling and crisis assistance, legal assistance, college preparation services, career development, housing supports, arts, sports and recreational activities, and nutritious meals – all for free and under one roof. By providing participants with our suite of integrated services, we seek to provide any motivated young person with the tools, resources, and opportunities needed to successfully transition to adulthood.
The Benefits Case Manager for Gender Expansive Youth will prioritize work with our Runaway and Homeless Youth (RHY) Department and the Adolescent Health Center (AHC). The Case manager will primarily work with Trans and gender non-conforming/gender expansive young people who are runaway and/or homeless/unstably housed. This role will specialize in entitlements, such as HRA benefits and social security, as well as vital documents support, including name changes and gender marker changes. They will also provide case management services such as crisis counseling, housing placement, mental health care, education, job/career assistance, recreational opportunities and other service needs are being met. This role will be a liaison between the Adolescent Health Center and RHY to provide direct referrals to trans-affirming medical care.
Responsibilities:
- Fieldwork and escorts to vital documents and benefits offices as required.
- Document all interventions with young people and on their behalf.
- Attend all relevant meetings, trainings, workshops and outside events.
- Provide suicide/homicidal assessments in collaboration with supervisor as needed to YP.
- Handle any appropriate responsibilities and functions as assigned by supervisor
- Assist young people to access public benefits through virtual support with online applications, on-site support at various sites across NYC (where applicable/permitted); provide advocacy, information and referral concerning health, housing, public entitlements, and other system issues or service needs; assist participants in the development and use of personal and community resources.
- Participate in case conferences for optimum collaboration among departments/programs and external agencies.
- Perform ACS mandated reporting duties as needed.
- In collaboration with other Door programs, especially the Benefits Engagement Specialist, identify young people in need of benefits and vital documents support.
- Demonstrate an understanding of and practice the principles of positive youth development.
- As the liaison to the AHC, provide direct referrals for trans affirming healthcare
Qualifications:
- Bachelor’s degree required
- Knowledgeable of and ability to apply positive youth development principles to working with young people
- Demonstrated ability in engagement and advocacy and collaboration on behalf of clients
- Effective and creative work in an inter-departmental and inter-disciplinary setting
- Proficiency in Spanish, French, or Mandarin preferred
- Experience with LGBT, Gender Expansive young people, and unstably housed youth as demonstrated by 1-2 years of direct service case management
- Demonstrated knowledge and experience working with adolescents and other city agencies
- Effective interpersonal and organizational skills
- Effective verbal, written and computer skills
Work Schedule: Full time, 35 hours a week, Tuesday - Saturday, 9:00 am - 5:00 pm/10:00 am - 6:00 pm
