Social Work Care Manager (Remote/Hybrid)

JOB SUMMARY
The Social Worker will be responsible for providing comprehensive care management services to our members enrolled in the Managed Long-Term Care (MLTC) program.

Requirements: Active LMSW/LCSW license in the state of NY (1 year of health plan experience; familair with Medicaid/Medicare)
Hours: Monday-Friday 8:30a-5:30p Pay: 74 - 84k
Location: This position is Mainly Remote, but will be going into the office for ~monthly meetings and training and may be asked to meet a patient at a provider's office or in their home if that is what is required for that patient.

ESSENTIAL DUTIES:
Conduct initial assessments and reassessments of members' needs, goals, and preferences.

Develop and implement individualized care plans that promote health, safety, and quality of life.

Provide ongoing care coordination and support, including monitoring of services and referrals to community resources.

Collaborate with interdisciplinary team members, including nurses, physicians, and other healthcare professionals, to ensure coordinated and effective care.

Participate in quality improvement activities to promote the delivery of high-quality, person-centered care.

Maintain accurate and timely documentation of all care management activities in accordance with regulatory requirements and organizational policies.
