Key Responsibilities

• Perform outbound and inbound calls, completing non-clinical pathways and screenings under guidance
• Coordinate with providers and Integrated Care Nurses to support Chronic Kidney Disease patients
• Help identify eligible resources for members and assist with enrollment in eligible programs
• Act as a single point of contact for DaVita IKC patients, providing ongoing communication and assistance
• Coordinate provider appointments on behalf of DaVita IKC members

Qualifications

• Medical experience is a plus
• Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook
• Preferred experience as a Patient Care Technician or Administrative Assistant in dialysis
• High School Diploma required
• Minimum of 2 years of experience in a professional office environment
