Description


Position Summary:


The individual in this position acts as the primary consumer advocate, keeps abreast of progress, and coordinates all services delivered to the consumer and family internally and through external referral.


Essential Job Functions


+


Conduct Needs Assessments


+


Develop Service Plans in collaboration with the consumer and family


+


Conduct monthly Service Plan Reviews


+


Document all clinical services and activities in accord with licensing, Medicaid, Joint


Commission standards, and agency policies and procedures


+


Conduct individual case management sessions, as needed


+


Perform activities on behalf of the consumer and family, including attendance at external appointments (e.g., court appearances, etc.), as needed


+


Provide the consumer and family with referrals and linkage to community-based services, as needed


+


Prepare monthly Progress Reports, as required, for assigned consumers for courts, probation officers, caseworkers, etc.


+


Coordinate consumer and family care with the treatment team and involved external providers


+


Responsible for the development of the discharge plan in coordination with other treatment team members


+


Arrange transportation for consumers, as needed


+


Assure implementation of and adherence to the behavioral management system, including providing behavioral consequences, motivational incentives, and learning experiences for the consumer


+


Participate in Case Conference


+


Provides transportation for consumers as needed.


+


Participate in meetings and attend trainings as required and requested


+


Understands HIPAA, Professional Ethics, Consumer Rights and Confidentiality


+


Participate in performance improvement activities


+


Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of


WestCare Foundation.

Perform any other duties as assigned.


Qualifications


Education:


+


Graduation from an accredited four-year college with a degree in Addiction Counseling,


Criminal Justice, Social Work, Psychology, or related field is required.


Experience And Competencies


+


A minimum two (2) years of related experience providing case management and/or care coordination services preferred.


+


Bilingual preferred.


+


Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is


+


required;


+


Must have familiarity of Broward county


+


Must have availability to work a flexible schedule including evenings and weekends.


+


Maintains confidential files and records.


+


Ability to identify and describe key signs, symptoms, impact and manifestations of trauma


+


Ability to prioritize and organize competing consumer and family needs


+


Ability to use trauma informed care principles including ensuring consumer safety, preventing/avoiding re-traumatization, maximizing trustworthiness, enhancing consumer choice, maximizing collaboration, and emphasizing empowerment

Aware of the differences between trauma informed and trauma-specific services


+


Demonstrates ability to work effectively with culturally diverse staff and populations


+


Demonstrates respect for individual’s cultural and ethnic values when conducting assessments, developing service plans, and providing services

Knowledge and application of psychopathology theory

Knowledge in the understanding of Motivational Interviewing, Motivational


Enhancement Therapy, Comprehensive, Continuous Integrated System of Care (CCISC), and Trauma-Informed Care
