Registered Nurse (Referral Coordinator) - Remote | Wfh

We are looking for a caring and experienced healthcare professional to join our team as a Clinical Triage Coordinator. In this role, you will have the opportunity to make a meaningful impact in the lives of our veterans by providing clinical triage of referrals and consults, coordinating with other specialties, and identifying barriers to care. You will also play a crucial role in ensuring that our veterans receive the most appropriate care option and modality of care by utilizing the facility's SLA and established CCAs.
