Gender Health Coordinator Jobs

The Gender Health (GH) Coordinator engages with internal and external providers to establish and maintain an evidence based, holistic, and supportive Gender Health services. The GH Coordinator will ensure that One Community Health interventions and team members are provided the tools to eliminate barriers to care and community connection for Transgender and Non-binary patients and families. The GH Coordinator is responsible for supporting the development, coordination, and training of One Community's GH program.

Essential Functions

• Leads development and implementation of site-based and agency-wide GH quality improvement initiatives.
• Develops and analyzes performance data reports for internal and external stakeholders and quality improvement initiatives.
• Identifies best practices, protocols, processes and trainings for health center staff and providers, in collaboration with HIV Program Director and Lead GH Clinician.
• Represents One Community Health in collaboration with external, clinical providers, administrators, and Quality Improvement leaders.
• Provides training, technical assistance, peer group facilitation and mentoring among GH staff.
• Manages GH services program reporting requirements for contracts in partnership with the Grants Department, Business Intelligence Team, site leadership, and frontline staff.
• Coordinates the development and implementation of a comprehensive GH program, outreach, treatment, and quality improvement strategy to support clinical providers and staff throughout the agency.

Additional Duties

• Provide care management amongst patients enrolled in GH program as back-up support.
• Manage program inbox within Epic, answer phone lines, chart patient notes, attend program meetings, and additional administrative duties as assigned.
• Other duties as assigned.

Required

Minimum Requirements

• Valid CPR/BLS Certification
• Care management in healthcare or non-profit settings
• Expertise in the following areas, typically gained from 4+ years in Gender Health services:
• Direct patient care serving LGBTQ+, homeless, BIPOC and other minority populations
• Baseline knowledge of Gender Affirming Care services, programs, and targeted outreach.
• Counseling, case management, motivational interviewing, and de-escalation amongst priority populations.
• Bachelor's degree in Gender Studies, Sociology, Psychology, Public Health, or related degree with two years working experience; or 4 years relevant experience required in lieu of education.

Preferred

• Bilingual in Spanish
• Experience with EPIC or other electronic medical record systems
