PPCCC is a trusted provider of high-quality, affordable reproductive health care along California’s Central Coast. Each year, PPCCC provides 35,000 people on the central coast with health care services; including sexually transmitted infection testing and treatment, birth control, breast and cervical cancer screenings, vasectomies, and safe and legal abortion care at our six health center locations. PPCCC also provides comprehensive sexuality education programs to help adults and teens make healthy decisions, prevent unintended pregnancies, and avoid sexually transmitted infection.


On-Site: The duties and functions of this job require the employee to work from a health center or office location every workday.


JOB SUMMARY:


A Patient Care Navigator provides case management services to patients in all aspects of sexual and reproductive health including, but not limited to abortion care, gender-affirming care, and lab follow-up. The Patient Care Navigator helps guide patients through the healthcare system and works to overcome obstacles faced by the patient while working to improve the systems and processes that shape access.


CARE NAVIGATION


Establishes trust with patients and serves as a primary contact.


Manages appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them.


Collaborates with the medical team including doctors, nurses, and other clinical staff to support positive patient outcomes.


Informs the patient on the financial aspects of care and links them to financial resources.


Identifies resources for patients to overcome barriers to care including but not limited to transportation, lodging, and legal support. Links patients to those resources as needed.


Ensures continuity of care by coordinating pre and post follow-up visits internally and with external medical providers.


CASE MANAGEMENT


Provides medically accurate information and gives patients five points of notification per PPFA standards, guidelines, protocols, and policies.


Makes all appropriate notification attempts including calls, letters, and patient portal messages for patient required follow up within PPFA-specified time frames.


Maintains and accurately documents in PPCCC EHR tickler system.


Maintains ongoing communication with Case Management RNs (CM-RN) and staff in health centers.


Reviews Visit Summaries, Med Master, and Order Module to ensure follow-up notification is accurate and appropriate for individual patient care and follow-up. Task CM-RN when an update or clarification is necessary.


Sends Confidential Morbidity Report (CMR) and/or electronic transmission of all positive HIV, GC, CT and RPR, and treated PID to Public Health Dept.


Completes medical records request for follow-up on outgoing and incoming patient referrals.


Other duties as assigned by Center Director.


Qualifications


Minimum of two-year experience in a health care support role or case management.


Experience and/or training in trauma-informed care, abortion care, and gender-affirming care, a plus


Excellent customer service skills and commitment to providing the highest level of customer satisfaction.


Must talk to patients in a caring and non-judgmental manner providing a positive patient experience.


Proficient computer skills in a Microsoft Windows work environment.,


Experience with Electronic Health Records NextGen is a plus.


English-Spanish bilingual skills desirable.


Planned Parenthood California Central Coast’s (PPCCC’s) compensation philosophy supports the organization’s mission, vision, and values. Each position has a salary based on market value and the organization’s pay levels. The starting salary for this position is $27.78/hour withwith an opportunity for an increase after six months of employment. As part of our commitment to pay equity, PPCCC does not negotiate salaries.


Pay Differential: $0.50/hour bilingual Spanish/English differential pay is offered upon passing the language assessment test.
