The Director of Care Management provides strategic direction, leadership, planning, organizational, and general management for all aspects of the Care Management program that includes Case Management, Post Acute program, Social Work, and Utilization Review.. This individual is responsible for development, implementation, and ongoing success of department and related organizational objectives and goals. Oversees through managers the accountability and stewardship of department resources and development of systems and procedures to impact operations broadly across the organization. Ensures case management standards are achieved in alignment with internal policies and external regulations.
