Case Management Coordinator Jobs

This position is work from home and will require home visits in the South Dallas, Ellis, Corsicana and Navarro Counties.

Travel will be 50-75% of the time.

Schedule is Monday-Friday, standard business hours 8a-5p CST. This is a non-exempt position

Utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources.

• Utilizes negotiation skills to secure appropriate options
• Coordinates and implements assigned care plan activities and monitors care plan progress
• Identifies and escalates quality of care issues through established channels
• Conducts comprehensive evaluation of Members using care management tools and information/data review
• Conducts multidisciplinary review in order to achieve optimal outcomes

and services necessary to meet the member’s benefits and/or healthcare needs

• Helps member actively and knowledgably participate with their provider in healthcare decision-making
• Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices
• Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health
• Monitoring, Evaluation and Documentation of Care: Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.

Pay Range
The typical pay range for this role is:
Minimum: 19.52
Maximum: 40.10

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Required Qualifications

• 2 years experience in behavioral health, social services.

• Willing and able to travel 50-75% of their time to meet members face to face for home visits in the South Dallas, Ellis, Corsicana and Navarro Counties and surrounding areas.

Preferred Qualifications

• Proficiency with computer skills which includes navigating
• Long Term Care experience preferred
• Managed Care experience preferred
• Discharge planning experience preferred

multiple systems and keyboarding

• 2+ years of Case Management experience
• Ability to multitask, prioritize and effectively adapt to a fast paced changing environment
• Effective communication skills, both verbal and written.

Education
Minimum of Bachelor's Degree or Non licensed master level clinician required, with either degree being in behavioral health or human services (psychology, social work, counseling.
