Discharge Facilitator, Case Management

Facilitates the transition of a patient and family from one environment to another by providing them with a comprehensive continuous plan of care, e.g., arranging transportation, preparing information to facilitate the patient's movement to the most appropriate level of care.

Minimum Education and Experience Requirements:

• Five or more years of related experience may be substituted for the credentials.
• Screening and assessment skills and expertise in care coordination and/or utilization management in a health care setting required.
• BSW, bachelor's degree or LPN required.

Licensure/Certification/Registration:

• Active and current appropriate licensure through the Department of Professional Regulation, State of Florida, as indicated.
