Job Title: Youth Case Manager
Department: Program, Youth and Teen Services
Reports To: Youth Case Management Coordinator
FLSA Status: Hourly, Non-Exempt

Background:
Good Samaritan Community Services is a 501(c) 3 organization that provides comprehensive services to more than 3,500 individuals and 1,200 families in various locations in South and West Texas. Since 1939 we have focused on serving the social, educational and health equity needs of our neighbors and our mission to "actively serve as a catalyst for change, supporting youth, individuals, and families by providing excellent community services to overcome economic poverty.
We do this through a continuum of services designed to stabilize families, establish nurturing relationships, and foster self-sufficiency in those we serve through the programs and supports we provided via our Family Services and Youth and Teen Departments. Good Samaritan began as a downtown mission of St. Mark's Episcopal Church in 1939 and was established as an institution of the Episcopal Diocese of West Texas in 1951 under the leadership of Bishop Everett Jones in 1951.
Position Summary
The Youth Case Management model uses a holistic approach that focuses on a child's mental, social, physical, or spiritual well-being. Utilizing the Strengths-Based Perspective, individual and group Case Management support. Travel is required for certification training provided to help youth build resiliency through social skill development (interpersonal relations, goal setting, conflict management, anger management, etc.). Services are designed to facilitate youth engagement, improve school and program behavior, and further foster relationships with youth and their families.
The role of a Case Manager is to encourage and support youth, connect them to resources, help them achieve self-sufficiency, and maintain an optimum level of wellness so they can engage in positive experiences at home, school, and in the community. Case Managers are responsible for working with youth to create an Individualized Service Plan that is used to track client progress and monitor individual outcomes. Through routine behavior monitoring and ongoing interaction with youth and their parents/caregivers, Case Managers can assess whether a youths needs are being met, and continually review and refine service plans as needed.
Required Education and Experience
- Experience with contract reports, budgets, and client management database systems preferred.
- Bilingual in English/Spanish preferred, according to business needs and requirements.
- One year of experience working with youth is required.
- Bachelor's degree in Social Work or related field with at least 12 hours of social work courses required.
Essential Duties and Functions
- Maintain detailed participant records and enter attendance, progress notes, and outcomes into the electronic data system in a timely manner.
- Provide case management in accordance with program policy and procedure.
- At intake, verbally orient parents/guardians to program rules, operations, and expectations.
- Monitor participant progress (behavior, development, and health status) and revise service plans, as needed and in tandem with the Youth Case Management Coordinator.
- Complete other duties as assigned by the Youth Case Management Coordinator.
- Support monthly parent meeting activities.
- Address immediate client behavior problems with Youth Case Management
- Deliver curriculum to program participants and develop lesson plans as needed.
- Demonstrate adherence to the CORE values of Good Samaritan Community Services.
- Link participant families to in-house or community services as identified in participant service plans.
- Confer with parent/guardian to address participant progress (academic, behavior, or health).
- Mentor and support participants to identify personal strengths and set and achieve goals.
- Coordinator, and assist staff in dealing with these behaviors, if possible.
- Demonstrate sensitivity to the social, physical, and emotional needs of program participants.
Supervisory Responsibilities
Manages volunteers and interns as assigned.

Physical Demands
While performing the duties of this job, one must be able to stand, walk, reach with hands and arms, stoop, kneel, talk, hear, type, have close vision, color vision, depth perception, ability to adjust focus with good hand-eye coordination, and occasionally lift between 25-35 pounds. These physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Expected Hours of Work
This is a full-time position. Days and hours of work are typically Monday through Friday, 9:30 AM to 6:30 PM. Hours may change based on agency needs.
Travel
Occasional local travel is required
