Overview Choices Case Manager is a professional who works with individuals and families to assess their needs and provide them with the resources and support they need to make informed decisions about their lives. The Choices Case Manager is responsible for providing case management services to clients, including assessment, planning, coordination, monitoring, and evaluation of services. The Choices Case Manager must be knowledgeable about the various services available in the community and be able to connect clients to the appropriate resources. Detailed Job Description The Choices Case Manager is responsible for providing case management services to clients, including assessment, planning, coordination, monitoring, and evaluation of services. The Choices Case Manager must be knowledgeable about the various services available in the community and be able to connect clients to the appropriate resources. The Choices Case Manager will assess the needs of clients and develop individualized plans to meet those needs. The Choices Case Manager will also provide ongoing support to clients and their families, ensuring that they are receiving the services they need. The Choices Case Manager will also provide advocacy and support for clients in their interactions with other agencies and organizations. Job Qualifications
• Bachelor’s degree in social work, psychology, or a related field
• At least two years of experience in case management
• Certification in case management or a related field
• Knowledge of relevant laws and regulations
• Ability to pass a background check
