The Youth Focus' HEARTH Beyond program provides rental assistance and case management services to youth ages 18-24 for up to 24 months. The Case Manager supports all youth in the program as well as works on site at My Sister Susan's House to provide support to youth in a residential setting, helping them learn independent living skills. The case manager is responsible for assisting clients with goal setting, providing case management, facilitating groups, supervising outings, providing transportation, and providing overall support in a therapeutic environment.

This position is Full time, Monday-Friday.

Must be able to attend an orientation and complete all onboarding requirements.

SUMMARY

The Case Manager must demonstrate the Agency's purpose, philosophy, policies and

procedures. The individual in this position will provide direction to youth who are transitioning to independent living.

The Case Manager identifies and implements individual case management plans for clients to ensure successful outcomes. Coordinates and follows up on services provided with the clients. Serves as an advocate and liaison for and on behalf of tenant's rights and benefits.

Reports to the Program Manager of HEARTH and My Sister Susan's House. This joint full-time position will consist of 40 hours a week Monday through Friday.

QUALIFICATIONS

• Minimum 21 years of age

• Bachelor's degree from a four-year college or university and at least two years' experience providing case management; or equivalent combination of education and experience.

• The ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.

• The ability to write routine reports and correspondence in a legible manner.

• The ability to use basic math skills including multiplication, division, understanding units of measure, using whole numbers, common fractions, and decimals.

• Must apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

• Must have a valid driver's license, at least three years of driving experience and a driving record in good standing

• Respects the cultural diversity of children in care, and strives to create a positive relationship.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Completes intake procedures as requested and organizes/develops service plans and utilizes professional skills so that clients make progress toward treatment goals
• Engages with tenants face-to-face and makes telephone or virtual contacts to ensure participation in programs
• Assists in coordinating evaluation of existing program components
• Provides integrated case management services that effectively engages tenants and promotes personal growth and wellness, housing stability, and accomplishments of service plan goals on an on-going basis.
• Collaborates with clients in developing a client-centered plan with a focus on health & wellness, housing stability and personal development.
• Facilitates and networks partnerships and linkages with progressive and/or alternative community agencies that serve the needs of Youth Focus clients, as collaborative member of the team, ensuring all clients have access to high quality services in a safe compassionate community.
• Effectively engages and works with a caseload of individuals in need of stable housing and other services.

• Works with other community agencies to make and receive referrals.

• Will visit clients in their homes and establish suitability living conditions through inspections. Will adhere to state rules and regulations.

• Creates and maintains confidential records, charts and logs for each tenant documenting all relevant information from individuals and group interventions as well as correspondences with collateral services.

• Enters confidential client data into database and maintains all data for HEARTH Beyond and MSSH

• Participates in staff team meetings, workshops, community meetings with tenants and attends off-site trainings.
• Documents all relevant information, referrals for actions taken in file, also includes; on-going follow up.
• Engages with the community to bring awareness to youth experiencing homelessness
• Engages and builds relationship with landlords in the community

• Receives instruction for daily activities from the program manager, coordinates this with residents' schedules and helps implement accordingly.

• Models for residents and gives them instruction regarding desirable health, personal habits, and independent living skills.

• Purchases food for meals and prepares meals as needed.

• Plans, leads, and participates in recreational activities

• Oversees the day-to-day activities of the program by enforcing the rules and policies of the maternity home and the agency. Supervises the residents and their children.

• Provides transportation, tutorial assistance, and emotional support to residents.

• Maintains any duties necessary to continue upkeep of the facility.

• Participates in team meetings and provides feedback to residents and staff about individual clients' progress and ways to improve programming.

• Represents the agency during community projects and business gatherings.

• Completes all required record keeping in a timely manner.

• Other duties as assigned by the Program Manager.

• Works under the supervision of the assigned supervisor.

Knowledge/Training

• Takes the necessary steps to stay current with all required trainings.

• Participates in all required in-service training.
