Shriners Children's is the premier pediatric burn, orthopaedic, spinal cord injury, cleft lip and palate, and pediatric subspecialties medical center. We have an opportunity for a Manager, Transition to Adulthood Program reporting into our Corporate Headquarters. The Manager, Transition to Adulthood Program is responsible for the overall development and execution, leadership, strategic vision and stewardship of the transition to adult hood care management program. This position is best suited for an experienced innovative, dynamic and engaging case management clinician who is self-directed, experienced in both program development and ongoing management of people and programs. This newly created role will require a system thinker who can successfully manage change and create an atmosphere of service excellence.


Program Development


  • Develop orientation materials to orient CM staff to this aspect of their role which might include SHINE modules and/or industry resources
  • Conduct workflow analysis and establish standard work processes for social workers and RN care managers for transition to adult care. Identify what are the key differentiators in the role responsibilities between the two roles.
  • Incorporates industry best practices into program to position Shriners Children’s as a best in class pediatric transition to adult hood program
  • Participate in discussions regarding new referrals over age 18 with key leadership decision makers
  • Use EPIC workflow as the guide to trigger assessments and interventions. Work with EPIC team resources to build assessments for staff to document work, questionnaires for adolescent to receive via MYCHART at age milestones, develop standardized educational materials to be pushed automatically at certain ages to adolescents and/or parents or guardian


Program Metrics and Monitoring


  • Develop and implement an audit tool to monitor and give feedback on opportunities during monthly meetings with leadership
  • Follow up with patient/family 6 months after transition to identify and evaluate opportunities for process improvement
  • Produce and analyze monthly reports by facility on the number of children at age milestones of age 12, age 14, age 15, age 16, age 17 and age 18, over 18 and review this data monthly with facility CM leader and DPCS and document and follow up on the intervention opportunities
  • Monitor diagnosis data and analyze for opportunities. Analyze patterns and make recommendations to leadership as appropriate.
  • Develop standard reporting package that will be sent to leadership team monthly on work being done


Transition to Adulthood Program Leadership


  • Collaborate with physician partners on opportunities to build community referral sources and on suggestions for program improvement
  • Represent the organization at conferences and in industry forums to highlight our work
  • Assists with transition coordination on difficult to place cases
  • Participate in warm handoff telehealth visits with care team
  • Serve as subject matter expert in the industry in the area of transition to adult care best practices
  • Recommend content for Shriner’s website and social media on transition to adult hood topics of relevance. Collaborate with marketing and communications teams
  • Work with parent advisory councils to get feedback on the ways we can support families
  • Orient new staff to the expectations for transition work at facilities and monitor for compliance with the program expectations for the first year of employment
  • Set clear expectations for work product and hold staff and leadership accountable for program results across multiple locations.


The ideal candidate will possess 10 years of clinical experience and expertise, as well as 5 years of Case Management experience (preferably working with adolescents and their families), Multi-facility system experience would be a plus. Current Masters Degree in Healthcare is required. Critical skills for this position include MS Office proficiency; interpersonal, negotiation and presentation skills; effective interdisciplinary team collaboration and consultation; proficiency in medical terminology and conditions with Shriners Hospitals for Children's scope of care; sensitivity and cultural awareness towards constituents from various countries and backgrounds; and ability to work well under pressure as well as strong organizational skills. Experience in case management, operations management, project management and strategic planning are key to this role.
