Community Nutrition Educator Jobs

The Community Nutrition Educator (CNE) will be responsible for managing the Nutrition Health Education (NHE) and Food and Meals Services program in accordance with AIDS Institute standards. NHE is provides monthly nutrition education and food vouchers to clients living with HIV/AIDS. The program is intended to enhance the knowledge and abilities of clients with HIV/AIDS to choose and prepare healthy safe meals; thereby aiding in preventing and reducing malnutrition and acute and chronic nutritional complications of HIV. In addition, the provision of NHE and food and meal services support retention in, and adherence to HIV medical care and treatment. The CNE will ensure program services and requirements are implemented.

Responsibilities

• Communicates and collaborates with AIDS Institute staff and responds timely and appropriately to all requests
• Conducts and edits monthly nutrition health education group workshops that incorporate HIV specific health and nutrition topics. Additionally, conducts individual education sessions for clients unable to attend group workshops.
• Plans and facilitates case conferences for each client every 6 months
• Reviews RD assessment and dietary recommendations to determine appropriate health education to meet needs
• Adhere to AIDS Institute requirements for the provision of Nutrition Health Education
• Oversees and performs accurate data entry for the program
• Actively engages with the Analytics & Evaluation department to request data reports and adjustments to support the program
• Distribute food vouchers to clients, monitor use, financial reporting to Director of Nutrition
• Enrolls clients to maintain client enrollment goals
• Acts as team leader with Nutrition Program Assistant to conduct program services
• Establishes connections and communicates frequently with medical nutrition programs/practitioners, and RDs; makes referrals as necessary and appropriate
• Submits monthly program data reports to Director of Nutrition
• Rescreens clients every 6 months to collect updated eligibility and health information, determine health needs and progress in the program
• Maintains client records that document attendance, participation, progress, accomplishments, and additional needs, referrals, and goals
• Ensures client adherence to NHE and food and meal policies and procedures, follows protocol for food gift card/voucher distribution
• Maintains up-to-date records across numerous databases and tracking sheets
• Attends all AIDS Institute required meetings and trainings.
• Monitors appropriateness of the food and meal service provided to the client
