Nutrition Coordinator, Patient Food Service, Essential Duties And Responsibilities

Support and promote patient satisfaction and participate with a multidisciplinary team to improve quality care/services to patients.

Communicate problems or concerns with patients to appropriate personnel in a timely manner, following department procedures.

Minimum two years experience working in a customer-facing support role.

Adhere to AIDS Institute requirements for the provision of Nutrition Health Education

Oversees and performs accurate data entry for the program

Reviews RD assessment and dietary recommendations to determine appropriate health education to meet needs

Submits monthly program data reports to Director of Nutrition

Actively engages with the Analytics & Evaluation department to request data reports and adjustments to support the program

Maintains up-to-date records across numerous databases and tracking sheets

Trains dietetic interns, other students, and health care professionals by planning, implementing, supervising and evaluating clinical experiences and/or teaching classes.

Conducts nutrition outcomes research in area of clinical practice and/or conducts or participates in collaborative nutrition investigational research.

The Nutrition Coordinator oversees the health and nutrition education services provided by all Local WIC Provider staff.

The Nutrition Coordinator reviews and approves all nutrition education materials (e.g., newsletters, recipes, bulletin boards, etc.)

Verification statement from a Dietetic Internship program accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND); or

A minimum of one-year professional experience as a dietitian or nutritionist preferred.

The Nutrition Coordinator plans and implements the nutritional components of the Local WIC program.

The Nutrition Coordinator monitors and prepares reports, records and other data related to nutritional services.

Provides accurate and timely documentation as required for Medicaid Waiver, Medicare, Social Security, ICF and licensing requirements

Read, interpret, implement, and understand policies, licensures, regulations, program requirements and state of the art methodologies of the

Ensure quality leisure activities and experiences for residents

Administers admissions and discharges of individuals

Coordinates and facilitates residential admissions and discharge committee meetings. Maintains candidate pool list and referral list

Coordinates with contracted and other providers to ensure individual needs are being met

Educate patients and their families on the importance of proper nutrition and dietary management in managing kidney disease.

Conduct initial nutrition assessments and follow-up evaluations for dialysis patients.

Develop and implement individualized nutrition plans for patients, taking into account their medical history, lab results, and current dietary habits.

Monitor and adjust patients' nutrition plans based on changes in their health status or treatment regimen.

Collaborate with the healthcare team, including physicians, nurses, and social workers, to provide comprehensive care for dialysis patients.

Monitor patients' nutritional status and provide recommendations for supplements or additional interventions as needed.

• Coordinate with case management team regarding resident volunteers

Occasionally provides clinical sales support by conducting inservices and educational programs with internal and external customers: staff, referral sources, payors, etc.

Assists with reimbursement related activities related to securing home HEN coverage, ie. Medicare qualification for home HEN services

Prepares electronic patient assessments and reports accurately and in a timely manner, as required by internal and external regulatory requirements

Reports to the appropriate manager(s) any known or suspected violations of policies and procedures, regulations or standards of conduct

5+ years of clinical nutritional support experience in a hospital or home infusion nutritional support team and/or critical care experience

Certification by A.S.P.E.N. Certified Nutrition Support Clinician-CNSC)
