Job Description

Pay Rate: $26/hr on W2


Must Have:


  • Comfortable providing a Photo ID Copy
  • Must have a Laptop/computer with internet to work from home.
  • Must be fully vaccinated (If not vaccinated, need an exemption letter)


Term of Employment

  • This position is 100% remote. The office is located in Baltimore, MD.
  • This position is full-time, 40 hours per week.
  • Contract-to-Hire, 6 months


Overview

Our client is seeking three (3) Assistant Care Managers to assist with member follow-up and coordination of care. In care coordination, there are various administrative and support tasks that need to be handled to ensure the smooth transition of patients after discharge from a healthcare facility or after completing their treatment (post-graduation). These tasks include activities such as monitoring the progress and well-being of patients after they leave the facility, scheduling follow-up appointments, arranging additional services or support they may require, and managing other administrative aspects related to their care.


Core Functions

  • Performs member or provider related administrative support which may include benefit verification, authorization management and case documentation.
  • Screens, and/or prioritizes members using targeted intervention business rules and processes to identify needed services. Transitions to appropriate clinical resources/programs as necessary.
  • Provides general support and coordination services for the department including but not limited to answering telephone calls, taking messages, letters and correspondence, researching information and assisting in solving problems.
  • Assists with reporting, data tracking, gathering, organization and dissemination of information.

  • Excellent customer service skills to interact effectively with patients, families, healthcare professionals, and other team members.
  • Need to have empathy and compassion towards patients and their families, understanding their needs and concerns.
  • Ability to make informed decisions based on patient needs, care plans, and available resources.
  • Ability to handle patient data and medical records with accuracy and confidentiality.
  • Proficient in Microsoft Suite.
  • Quiet work-from-home space/office.
  • Effective written and verbal communication.
