Job Description
We are a dynamic and compassionate organization dedicated to improving the lives of individuals and families. Our team is committed to providing exceptional care and support through our specialized programs. We are currently seeking a dedicated and driven Care Services Coordinator to join our team.
Position Overview
As a Care Services Coordinator, you will play a pivotal role in providing support, information, and resources to individuals living their caregivers, and families. Through various communication channels, including virtual visits, phone/email, in-clinic consultations, support group meetings, and home visits as appropriate, you will establish and coordinate a comprehensive plan of care. This role involves active management of a patient portfolio to ensure their needs are met while promoting a high standard of care and service.
Key Responsibilities
Receive referrals and gather essential patient, medical, and demographic information from referral sources.
Conduct specialized care consultations and needs assessments, addressing the unique requirements of patients and their families.
Provide information on available services, resources, and educational materials to patients and their families.
Coordinate referrals to Association programs and connect individuals with relevant healthcare providers and community agencies.
Facilitate resource support group meetings, ensuring participants' engagement and arranging informative speakers.
Collaborate closely with our Managing Director Care Services/Care Services Director and other care services staff to maintain strong relationships with multidisciplinary clinics.
Assist in evaluating the effectiveness of care services programs and make recommendations for improvements.
Ensure the consistent use of databases within your territory and adherence to established standard operating procedures.
Produce accurate and timely reports, including statistical and outcome metrics.
Work with our Managing Director Care Services/Care Services Director to establish strategic community and professional partnerships.
Assist in identifying and resolving operational and service barriers within your territory.
Willingness to travel as required or requested.
Perform other related duties as assigned by management.
Supervisory Responsibilities
This position does not currently have supervisory responsibilities. However, it is possible that supervisory duties may be added based on the evolving needs of the role and the Association. When supervisory responsibilities are assigned, the requirements are as follows:
Supervise remote and office-based care services staff in delivering programs and services to affected individuals and their families, as needed.
Collaborate with the Managing Director Care Services/Care Services Director in recruiting, coaching, mentoring, training, and conducting annual performance reviews for care services personnel.
Conduct regular one-on-one meetings with assigned care services staff, as applicable.
Qualifications
Bachelor’s Degree in social work, health administration, public health, nursing, or a related field (Master’s preferred).
Three to five years of related professional experience, including direct care, case management, and leadership or management experience.
Strong problem-solving and critical-thinking skills.
Knowledge is a plus, but not required.
Experience with progressive neurodegenerative diseases is highly valued.
Familiarity with Medicare, Medicaid, patient assessments, planning, and case management is preferred.
Excellent communication skills (interpersonal, verbal, and written).
Ability to handle confidential information with the utmost discretion.
Exceptional organizational skills and attention to detail.
Strong work ethic, personal integrity, and empathy.
Proficiency in Microsoft Office Suite; Salesforce and database experience is a plus.
Self-starter with the ability to work independently.
Excellent customer service skills.
Must have the ability to travel locally and nationally for Association business.
Flexibility to work occasional evenings and/or weekends for Association business and events.
