What you'll do...

The main duties involve conducting thorough chart reviews to verify assigned care gaps and actively engaging with both prospective and existing patients via phone. The goal is to assist interested patients in navigating the scheduling process, requiring a minimum of 60 outbound calls daily. Prioritizing calls based on health risk stratification is crucial, and efficient management of work queue desks ensures timely response to patient call back requests and follow-up calls, including appointment reminders. Detailed documentation of phone messages and patient interactions is essential for maintaining accurate records.

Minimum Qualifications

• Population health or valued-based care.
• Critical thinking; able to work alone and be self-directed.
• 2 years of experience making outbound calls as a Medical Assistant
• Problem-solution oriented.
• Negotiation skills
• Medical Assistant Certification
• 2 years of experience with chart review
• Ability to be empathetic.
• MS Suite proficient

• Organized and with strong communication skills written and verbal.
• EMR experience (i.e., EPIC system, E-clinical works, and Allscripts).
• Ambulatory experience
• Data entry experience
