Key Responsibilities
Empowering Support: Offer intensive case management services, conducting routine home visits, and maintaining regular phone contact to empower and guide our clients towards achieving their health and social service goals.

Requirements
Associate's degree in Social Work, Public Health, or a related field, demonstrating your foundational knowledge and passion for community health.
Experience with community health issues, showcasing your dedication to making a positive impact in people's lives.
Valid driver's license with a clean driving record, enabling you to travel within the Greater Philadelphia region for home visits and community outreach.
Preferred Qualifications
Bachelor's degree in Social Work, Public Health, or a related field, highlighting your commitment to professional growth and excellence.
2+ years of experience providing direct services in a community outreach setting, emphasizing your expertise in supporting maternal and child health.
