In role you will provide clinical nursing care and support to the Cardiac Surgery Program at UCLA Health. This full-time position will be providing direct patient care in the Cardiac Surgery Program. Responsible for providing clinical care to the patients and coordinating care with the team of physicians, nurse practitioners, clinical nurses, social work and administrative staff. The role entails providing direct and indirect patient services for the Cardiac Surgery population with a specific focus on remote monitoring for patients and transitions of care between the inpatient and outpatient setting, including telephone and inbasket triage for all medical and patient care issues. Salary Range: $60.79 - $91.97/hourly
* Bachelors or Masters in Nursing * California Registered Nursing License * CPR: BLS from American Heart Association or American Red Cross * 5 years RN experience (preferred) in outpatient or inpatient setting with experience in complex patient management. * Cardiac experience and/or understanding of cardiac surgery, strongly preferred. * Strong written and oral communication. * Ability to collaborate with outside providers/insurance plan administration to facilitate patient care for complex medical issues and to provide and assist with referrals to appropriate community resources to patients and their families, acting as a patient advocate. * Ability to work independently, organize and follow through on assignments with minimal direction. * Ability to triage patient care issues, prioritize when there are changes in workload and pressures of deadlines. * Ability to interface effectively with patients, staff, faculty, administrators, and colleagues. * Strong computer knowledge and ability to use word processing and database programs, including MS Word, Epic/Care Connect, and Excel. * Skill in developing a plan of care including setting goals, monitoring and documenting progress; ability to implement interventions that are pertinent to goals established in the plan of care * Ability to evaluate the effective of interventions toward the attainment of established goals and to recognize the need to reassess or reset goal parameters * Ability to delegate tasks in a management style that is consistent with promoting leadership and teamwork * Knowledge base to provide education to patients, families, and other staff regarding clinical and patient care issues
