Responsibilities:
Under the supervision of the Manager, Integrated Care Management (ICM), the Supervisor ICM is responsible for the daily oversight of Care Coordination teams providing clinical direction, organization, and monitoring of all care management services. Responsible for direct supervisory oversight of staff conducting care coordination, complex care management and other supportive functions. Assists in carrying out the plans and daily operations of the Care Coordination unit. Provides unit and departmental leadership. Ensures that staff receives appropriate orientation and training to organizational and departmental policies and procedures and ensures that staff have the clinical expertise to perform assigned duties. Works collaboratively with the Manager to prepare reports and conduct analysis of operations/services as required by departmental, corporate, and/or regulatory requirements. Participates in current process review and development of new and/or revised work processes, policies and procedures relating to Care Coordination responsibilities.
- Ensures that staff receive appropriate New Hire onboarding and training to organizational and departmental policies and procedures and ensures that staff have the clinical expertise to perform assigned duties. Participates in training.
- Performs other related duties and projects as assigned.
- Provides coaching and training to improve productivity within the scope of HR policies and procedures to all direct reports.
- Performs quality audits per ACDE policy for assigned staff. Shares audit results with associates and provides necessary education and counseling to improve performance.
- Provides clinical supervisory oversight and coordination of ACDE programs in accordance with applicable statutory provisions and complies with, NCQA, CMSA and DMMA requirements, and established policies and administrative procedures.
- Completes annual performance evaluations of all assigned staff. Completes evaluations using objective and measurable performance measures. Completes evaluations on time. Develops, implements, and monitors associates placed on performance improvement plans under the direction of the Manager/Director.
Education/Experience:
- Attention to detail, as well as excellent oral and written communication skills; good decision-making and problem solving skills, as well as the ability to prioritize and organize
- RN or MSW-LCSW preferred - MSW LMSW
- 1 to 3 years supervisory experience
- Active and unrestricted Delaware nursing license
- Proficiency with Microsoft Office Suite (Word, Excel, Power Point)
- 3 to 5years professional practice experience (inpatient/outpatient experience)
