Reporting to the Director of Pharmacy Services, UCLA HealthAmbulatory & Community Practices. Staff Pharmacist I performs a range offunctions to include: ensures maintenance of and adherence to establishedpolicies governing the safe, consistent and accurate dosing and administrationof medication. Evaluates clinicallysignificant drug-drug and drug-disease interactions, making appropriaterecommendations. Demonstratesproficiency in side effect, anticoagulation, hypertension, palliative care,motivational interviewing, symptom and pain management. In compliance with established policies,conducts medication reconciliation & prescription renewal through interfacewith prescribers and evaluation of lab and compliance data, including holidayon call coverage on a volunteer basis. Performs tasks associated withpopulation health initiatives including ordering of medications and requisitelab tests. Participates in and supports research activities and qualityinitiatives designed to improve patient outcomes. Maintenanceof accurate and appropriate documentation regarding patient encounters inEPIC/Beacon or other EMR. Well able toperform patient teaching, including evaluating barriers to compliance,suggesting positive resolutions, identifying therapeutic endpoints, andfollowing through. May oversee preparationand dispensing of conventional chemotherapy, support medications, immunemodulators and investigational drugs. Preceptspharmacy students, medical residents and medical interns. Conducts quality controlinspections and chart reviews necessary for ambulatory clinic certification. Participatesin budgeting, contracting, ordering, and return of pharmaceuticals. Other tasks or projects assigned per managerdiscretion. Is an effective, collegial,positive member of a multi-disciplinary health care team. Schedule: Monday - Friday; 8:00am - 5:00pm Hourly Salary Range: $80.94 - $94.81
Required · Current California Board of Pharmacy Pharmacist licensure in good standing. · PharmD degree. · Ability to set priorities which accurately reflect the relative importance of job responsibilities. · Skill to work independently and follow-through on assignments with minimal supervision. · Knowledge and ability to understand and interpret current scientific literature. · Effectively communicate orally using appropriate vocabulary and grammar to obtain information, explain policies and procedures, and to persuade others to adopt a specific opinion or action. · Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations. · Knowledge to handle or prepare intravenous admixtures and other sterile products such as intrathecal injections according to acceptable stands of aseptic technique. · Skill in writing articles/monographs/emails, which are concise, coherent, and use appropriate vocabulary and grammar. · Proficiency in all aspects of electronic medical record operation, particularly in the interpretation of laboratory and clinical data and physician recommendations necessary to authorize prescription refills. · Ability to organize, prioritize, and follow through for multiple patient treatments on an ongoing basis. · Ability to accept direction from leadership and act promptly. · Demonstrated ability to establish, nurture, and maintain cooperative working relationships with peers, co-workers, health care practitioners, and administrators. · Skill in listening perceptively, conveying awareness and responding to patients and visitors with health concerns using tact and discretion. · Ability to interact diplomatically and sympathetically with patients, their families, and the public in a high volume, clinical setting. · Skill in performing with frequent interruptions and/or distractions including the ability to accept equivocal circumstances and respond to changing priorities and deadlines · Ability to maintain confidentiality and to work with sensitive and confidential information using discretion, professionalism, and good judgment · Ability to provide evidence-based recommendations for symptom management. · Proficient in Care Connect/BEACON order entry system. Preferred · Previous clinical research experience. · Previous acute care, ambulatory care, or oncology patient experience. · Working knowledge of the Care Connect registration system, appointment-scheduling system. · Ability to operate automobile with appropriate licensure and insurance. · BPS Board certification. · Completion of PGY1, PGY2 residency. · Advanced Practice Pharmacist (APh) licensure.
