As a Clinical Services and Support Nurse, you will play a crucial role in transforming patients' lives by delivering exceptional clinical and educational services. Your focus will be on providing support to patients utilizing our innovative SmartMat to minimize the risk of diabetic foot ulcers. With your clinical expertise, evidence-based practices, critical thinking skills, and behavior change techniques, you will empower patients on their path to optimal health.
Responsibilities
Embrace a patient-centered approach, delivering superior care and support to each individual
Provide comprehensive training and education to patients on the proper use of the SmartMat, along with ongoing support and coaching to enhance their experience
Engage patients actively in the Podimetrics program, ensuring their continued participation and adherence
Facilitate seamless communication between patients and healthcare providers to enhance clinical outcomes
Conduct telephonic clinical assessments to identify any concerning findings that require immediate intervention, assess ongoing educational or coaching needs, and reinforce best practices for foot care
Utilize best practices to empower patients to self-manage their conditions, regain their mobility, and lead fulfilling lives
Provide valuable feedback and share the voice-of-the-patient experience to inform program development, process improvement, and technological advancements
Qualifications
Required:
Bachelor of Science in Nursing
Consideration given to Registered Nurses with a Bachelor's degree in a field other than nursing
Current state licensure as a Registered Nurse with no restrictions
Minimum Of Five Years Of Clinical Nursing Experience
Experience working in a fast-paced environment
Previous experience in remote monitoring or telehealth
Preferred
Understanding of and experience applying behavioral change techniques
Bilingual ability
