Summary of Duties:
Function as a member of the multidisciplinary health care team to facilitate patient movement through the continuum of care and back into the community. As part of the team you will be working collaboratively with the patient, patient representative, medical providers, social workers, nursing staff and therapy department in facilitating the highest level of patient centered care that will reduce the hospital length of stay while insuring the appropriate transitional plan of care back to the community. The social worker case manager will attend daily medical rounds with the interdisciplinary team daily and will collaborate with the team. Responsibilities include but are not limited to performing initial assessments within 24 hours of admission, implement the CHOICE Form for the patient and their representative to engage in customizing their discharge plan. Provides he appropriate Medicare letters to the patient on their hospitalization which are the HINN letters (Hospital Issued Notice of Noncoverage), MOON (Medicare Observation Outpatient Notice), IM Letters (Important Message from Medicare) and Detailed Notice of Discharge (DND) when indicated. The Social worker case manger will be responsible for the admit phone which includes assisting with the transfer process of patients to our facility and transfers out of our facility. The RN medical case manager will provide services to the patient and their representative with any social determents to reduce barriers to their discharge. This position requires a TEAM approach to the patient and the interdisciplinary team. This position requires effective and efficient communication skills, proper clinical documentation, professional code of conduct and critical strategic planning skills.
Provides interface with internal departments, external departments and agencies, and patients needing Social Service and/or RN Medical Case Management Interventions.
In addition to performing the essential functions listed below, may also be assigned other duties as required.
Educational Requirements: Bachelor's Degree in social work required MSW Preferred.
Licensure/Certification Requirements: Current AHA Basic Life Support (BLS) certification required. Licensed Clinical Social Worker (LCSW) required.
Experience Requirements: Experience in acute care hospital with diverse patient populations required. Knowledge of abuse reporting, Title 22, and Patient Rights regulations required. Ability to provide counseling for broad range of patients and families in culturally and economically diverse population required. Familiar with community resources required. 3-5 years Case Management and Social Work experience required.
Special Skills or Abilities: Proficient in Microsoft Office: Word, Excel, and PowerPoint preferred. Ability to communicate effectively orally and in writing, including report preparation required. Familiar with use of language line preferred.
