The Social Worker responsible for providing direct Social Work services to patients/families throughout the hospital setting. Assessing patient/families for Social Work intervention upon admission & throughout discharge.
- Communicate effectively (written and verbal) with admin, colleagues, patients and family members. Utilizes standard Social Work intervention techniques working with patients and families
- Resource to other staff members. Acts as a role model by selecting appropriate environment to express feelings and frustrations. Attends and actively taking part in the regularly scheduled Department Meetings
- Performs psychosocial assessments and interviews to develop safe discharge plans on complex patients. Supports and advocates for patients, makes referrals to appropriate agencies all while exploring financial alternatives, as needed.
- Provide ongoing case management support as necessary to the patients served at St. Mary's Hospital. Assesses patient's needs and plans care in accordance with the hospital’s policies/procedures and standards
Work Hours/Shift:
- Full-Time, 40 Hours with weekend rotation
Requirements:
- Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.
- Experience with utilization management, home care, community resources, and rehabilitation beneficial.
- CT State Clinical Social Work Licensure MSW required, (LCSW) preferred
- Education: MSW degree from an accredited School of Social Work required.
