life. We provide individual, group, and couples therapy to adolescents

and adults. Within our practice we seek to address a range of emotional

and behavioral issues providing services that span from therapy for

depression and career counseling to relationship concerns, family

conflict, and beyond. In a comfortable and supportive environment, we

offer a highly personalized approach tailored to each of our clients

individual needs to help them confront challenges, attain the personal

growth they're striving for, and become the best possible version of

themselves.

The Health Coach assesses the participant’s current needs and readiness to change to deliver appropriate education, support, and guidance to facilitate healthy lifestyle change to prevent or manage chronic conditions.

Additionally, the Health Coach is responsible for assisting participants to explore personal “best health” vision and purpose, set aligned SMART goals, encourage progress, and overcome barriers which may interfere with reaching

established goals. The health coach facilitates communication between the patient, family, and members of the health care team while acting as an educator and link between the patient, providers and the plan, ensuring high quality, cost-effective services are delivered. Helps members coordinate care and navigate the healthcare system.

Requirements

• Masters in a health related field or equivalent experience (preferred).
• Able to effectively balance attention to detail with efficient execution in a startup environment that's
• As a coach, you are naturally empathetic and easily create camaraderie with your coaches while remaining objective and able to guide them through processing whatever may come up in a session.
• Strong writing skills, tone, spelling and grammar, appropriate to our messaging guidelines.
• Excellent communication and critical thinking skills with high attention to detail and quality.
• Certified or have the ability to complete an approved certification within 18 months of hire (Wellcoaches preferred).
• Strong ability to empathize with chronic pain sufferers.
• Embody high emotional intelligence with a patient- centered, eager-to-help approach.
