Job Description
General Description Of Position


The Manager for the Population Health Department leads the Population Health Care Coordinators and Nurses. They handle all of the administrative personnel duties and manage the department. They, and their teams, collaborate with physicians, hospitals, patients and post-acute facilities to provide excellent outcomes. The Manager of Population Health will also spend time networking in the community to build new relationships with physician offices and primary care physicians.


Essential Duties And Responsibilities


  • Works with team to develop and implement department policies and procedures.
  • Works alongside their team to plan, implement, and evaluate patient’s health needs.
  • Perform any other related duties as required or assigned.
  • Develops call scripts for coordinators/nurses to utilize for patient care.
  • Monitors cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
  • Assist in patient care by assessing patient needs and developing a plan to help patients succeed after surgery.
  • Work with physicians to develop patient optimization and ensures ongoing care for patients is met.
  • Prepares clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
  • Participates in the assessment of staffing needs.
  • Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends.
  • Assist patients with navigating through the health system and is used as a resource for support and follow-up.
  • Attends required meetings and participates in committees as requested.
  • Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
  • Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
  • Identifies, studies, and recommends solutions to staff related issues.
  • Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
  • Communicates updates to staff on changes in workflow, policies, procedures, etc.
  • Completes the hiring, training/orientation, and performance evaluation of assigned staff.
  • Maintains time and attendance records of staff. Approves time reports and overtime, and coordinates scheduled paid leave of assigned staff.
  • Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
  • Determines clients' requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
  • Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.
