Diabetes Educator - Remote | Wfh

We are seeking a passionate and dedicated healthcare professional to join our team as a Diabetes Self-Management Educator. In this role, you will have the opportunity to make a meaningful impact on the lives of individuals, families, and groups by providing education and training in diabetes self-management and medical nutrition therapy.
You will play a vital role in the care of our patients. You will assess their educational needs, develop personalized nutritional guidelines, set lifestyle intervention goals, and provide counseling to empower them with the knowledge and skills to effectively manage their disease. Collaboration with other healthcare providers, including physicians, pharmacists, case managers, and social workers, will be essential in developing individualized protocols encompassing medication, therapy, nutrition, and devices.
Experience
Minimum of 2 years of recent relevant experience
Skills And Knowledge
Proven competency in the field of diabetes education, including clinical judgment, technical skills, and a strong knowledge base
Expertise in using diabetes monitoring and managing equipment, such as blood glucose monitors and insulin injection devices, as well as associated computer software
Advanced understanding of adult learning theory and the ability to provide individual and group education
Exceptional attention to detail, with excellent time management and organizational skills
Proficiency in computer applications, including Microsoft Office Suite and EPIC for electronic medical records (EMR) management
Ability to prioritize tasks, make effective decisions, and delegate responsibilities
Problem-solving skills to address atypical or infrequent issues, providing guidance and support to junior team members
Ability to work both independently and collaboratively in a fast-paced environment with interruptions
Excellent communication skills, with the ability to convey technical or sensitive information clearly and professionally to diverse audiences, including patients, their families/support groups, physicians, and staff
