Requires assessment skills, organizational and time management skills, the ability to collaborate with community partners and citizens of Forsyth County.

Knowledge of social work principles, techniques and practices and their application to specific casework and the community.

Knowledge of Special Assistance/In Home program

Knowledge of behavioral and socioeconomic problems and their treatment and governmental and private organizations and community resources.

Knowledge of the laws, regulations and policies that govern social work programs.

Ability to express ideas clearly and concisely and to plan and execute work effectively.

Provide adult services case management for eligible recipients.

Provide other case management functions as assigned

Develop and implement individualized person-centered plans consistent with DHS requirements.

Maintain and apply knowledge of relevant policies, procedures, and statutes and professional practice.

Assess and determine eligibility for Rule 79 MH-TCM and provide short-term MH-TCM.

Assist individuals in obtaining necessary evaluations to determine eligibility for services.

Carry a maximum caseload of 9-10 young adults

Provide on call crisis support to the client 24 hours a day 7 days a week (alternating on-call duties).

Provide accurate and complete information to Clinical Supervisor in a timely manner in the event of a crisis situation.

Participate weekly in all supervision exercises to ensure the appropriate conceptualization and implementation of treatment.

If a new graduate Social Worker, Limited Permit (must supply exam date prior to start of employment)

Recruit, manage, and train referees, coordinators, and all other programming personnel

Successfully manage all nightly program events

Working knowledge of all major sports

Previous Sports Officiating, Coaching, or Playing Experience

College Degree preferred, but not required with relevant experience

Coordinate and referee games as needed
