Social Worker - Adult Services

The Forsyth County Department of Social Services is seeking an Assisted Living Services Social Worker for the Adult Services Division. The Social Worker responsibilities include but are not limited to: conducting assessments, developing service plans, collaborative services and communication with providers to ensure the customers needs are being met. For In-Home Aide services the Social Worker will be responsible for monitoring the customers services provided by In Home Aide Subcontractors (i.e., home health agencies). Additionally, this position will maintain electronic files; therefore computer skills are a must.

• Knowledge of the laws, regulations and policies that govern social work programs.
• Requires independent decision-making, assessment skills, professional judgment, effective interview skills, ability to collaborate and knowledge of community resources, ability to effectively communicate (both orally and written) with a diverse population from a variety of socioeconomic backgrounds and a variety of physical and/or mental capabilities, must be able to multi-task
• Requires assessment skills, organizational and time management skills, the ability to collaborate with community partners and citizens of Forsyth County.
• Knowledge of Special Assistance/In Home program
• Ability to express ideas clearly and concisely and to plan and execute work effectively.
• Knowledge of behavioral and socioeconomic problems and their treatment and governmental and private organizations and community resources.
• Knowledge of social work principles, techniques and practices and their application to specific casework and the community.

Bachelor's degree from an accredited school of social work or;

Master's degree in counseling or;

Four-year degree in a human services field or a related curriculum including at least 15 semester hours in courses related to social work or counseling and six months of social work or counseling experience or;

Four-year degree and one year of experience in counseling or in a related human services field which provides experience in techniques of counseling, casework, group work, or community organization.

An equivalent combination of education and experience may be considered for minimum qualification requirements.

Valid driver's license is required. County vehicles will be used for conducting home visits, transporting clients to and from medical appointments and running essential errands.
