Job Description
Position Summary
Under general supervision Behavioral Health uses extensive knowledge and skills obtained through education and experience to provide a wide array of social work functions such as assessment interviews, client counseling, case evaluation, and/or other social services to individuals, groups or families in a behavioral health setting. The employee may provide referrals to appropriate community services, including financial assistance or healthcare. The individual provides accurate and understandable interpretations of program policies and regulations to clients and maintains proper records according to the organization, state, and federal guidelines.
Responsibilities/Accountabilities
Essential Duties and Responsibilities:
  • Maintain patient confidentiality.
  • Involve the patient in identifying and understanding presenting problem.
  • Maintain patient confidentiality by following protocol and procedures set for HIPAA guidelines
  • Establish a treatment plan that provides maximum benefit to the patient.
  • Responsible for contributing to the patient team care approach as it relates to social worker supervisor/behavioral health care worker duties.
  • Abilities:
  • Provide for a multi-disciplinary approach to a treatment plan, coordinating with other staff.
  • Assess patient progress and modifies the treatment program accordingly.
  • Maintain documentation on all case files including patient information, treatment plan, progress and outcomes.
  • Participate in professional development activities and maintains professional affiliations.
  • Educates and support the patient regarding options and services available.
  • Coordinate care of patients with referring health provider(s) or agencies.
  • Consult with medical/referring providers integrating behavior and primary health.
  • Attend and participate in conferences to discuss patients' care and other staff meetings. Participates in committees as requested.
  • Assist clients with interpreting when needed.
Knowledge Of
  • Community resources including social services agencies and crisis intervention providers
  • Intermediate to advanced concepts, practices, and procedures of crisis intervention techniques
  • Processes, rules, and regulations of various services and funding agencies surrounding the eligibility and maintenance of the services being received
Skill In
  • Operating standard office equipment and using required software applications for the program area and other applications, including Microsoft Office, Michigan Single Sign On, and Community Health Automated Processing System (CHAMPS) based on assigned functional area

  • Handle stressful situations and provide a high level of customer service in a calm and professional manner
  • Maintain confidentiality of agency and client information.
  • Speak and write in English.
  • Organize and prioritize multiple tasks and meet deadlines
  • Communicate effectively, both orally and in writing.
  • Partner with other functional areas to accomplish objectives
  • Objectively gather information, identify linkages and trends, and apply findings to assignments
  • Facilitate meetings, ensuring that all viewpoints, ideas, and problems are addressed
Educational/Previous Experience Requirements
  • Bachelor's degree
